Saint Peter’s Healthcare System physicians and Partners In Care expand Relationship to include participation in the Partners In Care medicare ACO for 2015

 

East Brunswick, NJ – Saint Peter’s Healthcare System and Partners In Care, New Jersey’s oldest physician-owned provider network and healthcare management company, finalized an innovative agreement for the hospital’s adult medicine primary care and multi-specialty employed physicians and extenders to join the Partners In Care Medicare ACO program for 2015. 

Under this arrangement, Saint Peter’s in-house physicians will have access to the Partners In Care ACO full suite of services designed to improve the efficiency and delivery of care for the unmanaged Medicare population. Value-added services include, but are not limited to, claims data analysis, individualized risk stratified patient action planners designed for a quality-based collaborative care approach including cost containment opportunities. Providers will also benefit by attending two-way sharing educational workgroup sessions intended to maximize collective efficiencies to provide stronger patient centric care to this senior population. They will also receive additional revenue for care coordination activities and population health management, which have proven track records in measurably improving patient health while lowering costs for the unmanaged Medicare population. The program payment model also includes shared savings potential.

This partnership is particularly remarkable in that Partners In Care was originally founded in 1995 as a joint venture between Saint Peter's University Hospital and its physician network, United Medical Group.  In 2005, Partners In Care transitioned to physician-only ownership.
