Faith Based Wellness Program

This Community Health Services program is designed to assess and address the health needs of members of faith-based communities. It has been demonstrated in evidence-based studies that faith-based wellness programs prove to be very successful in improving health because members hold great trust in their faith and spiritual leadership.

The Community Health Services Department works closely with members of faith based communities to coordinate health fairs at faith-based centers. Members of the department provide Health fairs consisting of various health screening services and education. Registered nurses and advanced nurse practitioners also provide follow-up services to participants.
Services provided at fairs can include:
- Blood sugar screenings
- Blood pressure screenings
- Cholesterol screenings
- Osteoporosis screenings
- Vision screenings
- Audio screenings
- DermaScan facial screenings
- Immunizations
- Referrals for community healthcare services
- Follow-up health care services
- Health Education;
